Cancellation of the contract - return within 14 days (Please fill in this form and send it back only if you want to cancel the contract)

Please help us to fulfil your wishes and improve our products by enclosing this completed form with the products you have purchased in the Evitas Shop and wish to return. Please also enclose the original invoice that we have sent you by e-mail. When processing your return, Evitas Shop d.o.o. will observe the General Terms and Conditions.

Sending the form counts as signing the contract for the return of the goods within 14 days (Termination of contract).
